The Lending Club Patient Financing

Dental Insurance

Smile Savers Dentistry accepts most insurances. This means the insurance companies submit payment for covered services directly to our practice. However, even if we are “out of network” with your particular insurance provider, we will still submit all dental insurance claims for you and help you walk through the process so they can reimburse you directly.

Please contact us for details on the insurance providers we work with and how we can help you obtain reimbursement from other providers. Patients remain personally responsible for their costs.

Appointment Cancellation Policy

Your dental appointment time has been reserved specifically for you. We request notification 48 hours before your appointment if you must cancel. But we know that unforeseen events or emergencies can make it impossible to keep a scheduled appointment. After a second appointment is missed without notifying us 48 hours in advance, a fee may be charged.

Program Guidelines:

Patients’ portion of bill is due at time of service. There will be a $50.00 reinstatement fee if your plan lapses.

Plan cannot be used in conjunction with any insurance that we are contracted with.

No refunds of premiums will be issued at any time.

If the Participant decides not to utilize this membership plan, it is non refundable.

Exclusions/Limitations:

This program is a discount plan; not a dental insurance plan. It cannot be used:

In conjunction with any dental insurance plans.
For treatment which, in the sole opinion of our doctors, lies outside of the realm of their capability.
For referrals to specialists.
For hospitalization or hospital charges of any kind.
For costs of dental care which are covered under automobile medical or workers comp.

You will not be singled out for rate increases or cancellations. You will not receive a membership card. Your plan’s effective date will be on file with Smile Savers Dentistry.
